Drug Test Authorization Forms are sent to the email address provided within 24 hours after submitting your order. Please note that after order submission, all sales are final. Donors will need to bring their authorization forms to the collection site in order for the test to be performed. Using the zip code provided, we will order your test using the closest collection site to you. We recommend calling the collection site to verify their exact drug testing hours and location. Many sites will be happy to schedule an appointment for you if needed. Their phone number and address can be found on your drug test authorization form.
